Ohio Medicaid publishes its own rule for these products, and it is the first thing below. A payment count is not a coverage rule, so the federal record of what the program paid sits under it, and this page says which it is showing you.

What Ohio Medicaid publishes as the rule

Ohio Medicaid excludes all 7 of these products for weight loss, effective 2024-02-16. The rows below give each one. Where the same molecule is sold for type 2 diabetes, that is a different use with a different answer, and those rows are listed under the table.

This is the published rule, not the payment record. Ohio writes its drug exclusions by what a drug is used to treat, so every row below answers for one use only: the same molecule sells under one brand for weight management and another for type 2 diabetes, and an answer for one is not an answer for the other. The payment file further down counts prescriptions the program paid for and never records why any of them was written, so it can neither confirm nor contradict a rule. The rule read here is the department's own pharmacy rule plus the managed care rule that points the single pharmacy benefit manager at it. Ohio's own list of covered drugs was not read for this record, because every path at the address the rule names answered not-found and the copy in the state portal sits behind a sign-in, so nothing here says what the program does for the diabetes use of these same molecules. MyCare Ohio and OhioRISE members sit under chapters this record did not read. None of it can tell you what will happen to your own claim.

Who actually pays this claim

Ohio did not move the drug benefit out of managed care. It put one administrator in front of all of it. State law directs the medicaid director to select a single pharmacy benefit manager for the managed care organizations, and that administrator processes every pharmacy claim in the managed care system. So the plan on a member's card is not what settles the drug rule, and there is no plan-by-plan drug list to compare.

A managed care member and a fee-for-service member meet the same drug rule. The managed care rule sends the single pharmacy benefit manager to the department's own pharmacy rule for prescribing and for prior authorization, and that pharmacy rule is written for drugs covered by the department's program or by a managed care entity. One limit is on the face of the managed care rule: it says it does not apply to MyCare Ohio plans or to the OhioRISE plan, and the chapters governing those two were not read for this record.

The rule and the payment record disagree, and we do not know why

The rule and the federal payment file point in different directions here, and this record does not settle why. Ohio puts drugs used to treat obesity outside coverage and says a drug in that category cannot be prior authorized, and the payment file below still records paid prescriptions for these products in every quarter it publishes. That file records what a pharmacy dispensed and never records why the prescription was written, so it cannot say which use any of those prescriptions was for. A prescription written for a use the obesity category does not reach, a claim decided under a review this record did not read, and a later restatement by the federal publisher would each leave the same pattern. Nothing read here chooses between them, and no count in that file is permission to fill the next prescription.

This record counts prescriptions Ohio's Medicaid program paid for, quarter by quarter, as reported to the federal government. It is not a coverage rule and it is not a promise. A count can be low because the program pays for few, because the reporting cell was too small to publish, or because most enrollees are in managed care that reports separately. A missing or hidden cell is never a zero, and the coverage rule lives in the state's own drug list rather than here.

How these figures are counted

Figures are for 2026Q1 and combine both payment routes, the claims the program pays directly and the claims its managed care plans pay. Thirteen jurisdictions report only the first and eighteen report only the second, so combining them is the only way one table can be read across the country.

What this page does not tell you

It does not tell you the criteria. The rule at the top of this page says what Ohio Medicaid covers and what it excludes; it does not carry the checklist a prescriber has to satisfy to get a request approved for a use that is still open. That document is published separately and this site cites it as it reads it rather than summarising it from memory.

It also does not tell you which condition a paid prescription was written for. The same molecule is sold under one brand for type 2 diabetes and another for weight management, and a program can pay for one while excluding the other on the same day. Where a distinction like that matters, the record carries the indication and the page says so.

Drugs that fall into one of the following categories are non-covered by the Ohio medicaid pharmacy program: (1) Drugs for the treatment of obesity.
The opening of paragraph (B) with the first of its seven categories, read from the rule page at the state's code site on 2026-08-09. The category is written by what a drug is used to TREAT rather than by product name, which is why it reaches a product the list never spells and why it stops at the boundary of that use. The categories that follow are infertility, erectile dysfunction, drug efficacy study implementation drugs, drugs eligible under Medicare Part D for a recipient eligible for Medicare, some over-the-counter drugs, and use for an indication the food and drug administration has not approved.

The second half of the opening sentence of paragraph (D), read from the rule page at the state's code site on 2026-08-09. The first half says a drug that is not on the covered list and is medically necessary requires prior authorization, and this clause is the carve-out from it. Obesity is the first of the five paragraphs the clause names, so the same rule that puts the category outside coverage also says the usual route around a coverage decision is not open on it.

Common questions

Does a paid claim mean Ohio Medicaid covers the drug for weight loss?

No. A paid claim shows that a prescription was filled and paid for. It does not say which condition it was prescribed for, and the same molecule is often paid for diabetes while being excluded for obesity. The program drug list and its prior authorization criteria decide the rule.

Why does a number say "at least"?

Because a reporting cell that is too small to publish is withheld rather than printed. When any matching cell was withheld, the total shown is a floor and the real number is higher. Printing that floor as if it were a count would be a false number with a citation attached to it.

What does an empty row mean?

It means no paid claim for that product appeared in the published federal file for that quarter. It is not a zero and it is not a coverage decision. Every empty row on this page carries the query that returned nothing and the date it was run, so you can check it.

Where does the rule actually live?

In Ohio Medicaid's own published documents, and the top of this page is where they are quoted. Each row there names the document it was read from, the section, and the sentence, so you can open the same page the reader of this site read.
