Evan Reid is the founder of Covered Weight. His role is research and sourcing: he reads published payer policy documents, meaning Medicare National and Local Coverage Determinations, state Medicaid preferred drug lists and provider manuals, and commercial medical policy bulletins, and records what each one says with its policy number, effective date, and the date he read it. He is not a physician, a pharmacist, a nurse, or an insurance broker, and Covered Weight does not provide medical advice or insurance advice. The authority on this site comes from the cited policy documents, not from him.

The documents behind this site are administrative rather than clinical: national and local coverage determinations, state preferred drug lists and provider manuals, benchmark plan charts, and commercial medical policy bulletins. Reading them accurately, recording what they say without smoothing it, and citing the line it came from is the whole job.
